Program Overview: The School-Based Early Support Program is a collaborative effort with ACS aimed at enhancing family empowerment and promoting the well-being of children within educational settings. This initiative provides a spectrum of services directly accessible to families through partnerships with local educational institutions.

Role: Family Support Case Manager

Reporting To: Program Manager

Responsibilities:

  • Engage families through outreach initiatives, including self-referrals, to discuss program services and expectations. This outreach prioritizes connections with partner schools and extends to community-based organizations to ensure comprehensive family support.
  • Work collaboratively with referral sources, including educators and community organizations, to understand each family's unique strengths and needs, ensuring appropriate service alignment.
  • Develop tailored support plans in partnership with families, based on thorough assessments of their strengths and service requirements.
  • Effectively manage a caseload of 12 clients.
  • Maintain precise documentation, including reports and databases.
  • Contribute to program evaluation through report preparation and participation in case conferences.
  • Recommend case closure in accordance with established guidelines.

Qualifications:

  • A Bachelor’s degree in a relevant field such as Social Work, Psychology, or a related discipline.

Additional Requirements:

  • At least two years of experience working with families and children in community settings.
  • Strong interpersonal and communication skills, demonstrating respect and empathy towards clients.
  • Exceptional writing abilities for clear and professional documentation and communication.
  • Excellent organizational skills with the capacity to prioritize tasks effectively.
  • Proficiency in Microsoft Office Suite and familiarity with case management systems is advantageous.
  • Commitment to cultural competency and anti-oppressive practices.
  • Understanding of student support services, including IEPs and SEL programs.
  • Fluency in English and Spanish or Haitian Creole is preferred.

Compensation: The annual salary ranges from $47,000 to $52,000, reflecting various factors unique to the role and candidate qualifications.

Employment Status: Full-time (35 hours per week)
