Position Overview:
We are seeking a qualified individual for the role of Infusion Case Manager - Oncology Services. This position plays a crucial role in coordinating patient care, focusing on services that extend beyond the hospital environment.
Key Responsibilities:
- Coordinate care for patients, emphasizing navigation through chronic disease management and fostering interdisciplinary collaboration.
- Assess healthcare needs and identify areas for improvement, implementing effective interventions.
- Manage transitions in patient care, ensuring a seamless experience for those with complex health conditions.
- Provide education on disease management and promote evidence-based healthcare practices.
- Collaborate with various healthcare professionals to ensure high-quality outcomes for patients and their families.
- Identify and address discharge planning issues, working closely with social workers and other representatives to meet patient needs.
- Monitor patient progress and communicate effectively with the medical team to ensure comprehensive documentation of clinical status.
- Utilize electronic medical records to facilitate care coordination and maintain compliance with established protocols.
- Engage patients in managing their health, medications, and follow-up appointments, ensuring they are informed and proactive in their care.
Qualifications:
To be considered for this role, candidates must possess a Bachelor's Degree in a relevant field. A combination of experience and educational credentials may be accepted in lieu of a degree. Candidates should have a minimum of five years of relevant clinical experience, with demonstrated leadership capabilities. Experience in Quality Improvement, Utilization Management, or Case Management is preferred. A valid New York State Registered Nurse License is required.
Preferred Qualifications:
Candidates with recent clinical experience, particularly in leadership roles, will be given preference. Additional certifications in Case Management and Basic Life Support (BLS) are also advantageous.
