The Case Manager plays a vital role in the supportive housing program, ensuring that tenants receive the necessary resources and guidance to thrive in their living environment. Key responsibilities include:
- Evaluating and maintaining records for program eligibility
- Performing intake evaluations and creating personalized independent living strategies
- Supporting tenants in achieving their personal goals and providing necessary referrals
- Championing tenants' self-sufficiency and overall well-being
- Tracking and documenting tenant development
- Engaging in case discussions and follow-up activities
Essential Qualifications:
- A Bachelor's degree coupled with experience working with individuals with Serious and Persistent Mental Illness (SPMI)
Additional Requirements:
- Successful completion of background checks and maintenance of fingerprint clearance
- Mandatory reporting of any incidents of abuse or neglect
- Availability for occasional flexible working hours
Salary Range: $40,000 - $45,000 annually
