The Case Manager plays a vital role in the supportive housing program, ensuring that tenants receive the necessary resources and guidance to thrive in their living environment. Key responsibilities include:

  • Evaluating and maintaining records for program eligibility
  • Performing intake evaluations and creating personalized independent living strategies
  • Supporting tenants in achieving their personal goals and providing necessary referrals
  • Championing tenants' self-sufficiency and overall well-being
  • Tracking and documenting tenant development
  • Engaging in case discussions and follow-up activities

Essential Qualifications:

  • A Bachelor's degree coupled with experience working with individuals with Serious and Persistent Mental Illness (SPMI)

Additional Requirements:

  • Successful completion of background checks and maintenance of fingerprint clearance
  • Mandatory reporting of any incidents of abuse or neglect
  • Availability for occasional flexible working hours

Salary Range: $40,000 - $45,000 annually
