FACILITY RELATIONS COORDINATOR

POSITION SUMMARY:

Under the guidance of the Quality Manager, the Facility Relations Coordinator will provide education and oversight to external facilities and their personnel regarding the PACE model, policies, and procedures. This role is essential in fostering a collaborative partnership between PACE Southeast Michigan and various external stakeholders, including nursing homes, assisted living facilities, group homes, dialysis centers, and contracted service providers. The Coordinator plays a vital role in our Interdisciplinary Team, contributing valuable insights that enhance our admissions process, ensure accurate financial reporting, and elevate the quality of care and service excellence in our relationships with partner facilities.

KEY RESPONSIBILITIES:

  • The Coordinator will be assigned a minimum of 18 facilities and will conduct monthly evaluations of 10% of participants admitted to these facilities and enrolled with PACE Southeast Michigan. Monthly reports will be compiled using provided data.
  • Establish and maintain strong working relationships with both internal and external staff, delivering educational sessions to contracted providers on a monthly or quarterly basis regarding PACE Southeast Michigan.
  • Conduct annual oversight that includes training on PACE Southeast Michigan, which encompasses:
    • Competency assessments for staff (5 Full-Time Employees) with background checks
    • Basic Life Support training with valid BLS certifications
    • CLIA verification
    • Facility oversight reviews
  • Serve as the primary contact for all matters related to contracted facilities, facilitating communication between PACE Southeast Michigan's Interdisciplinary Team and external providers regarding participant appointments, care conferences, and other collaborative meetings.
  • Perform general rounds on all participants, documenting concerns and reporting them to the appropriate team members for resolution.
  • Act as the main channel for communication between contracted facilities and PACE Southeast Michigan, ensuring clear and effective exchanges both verbally and in writing.
  • Participate in Interdisciplinary Team meetings as needed, providing timely MDS scores to the finance department.
  • Document and escalate all facility-related concerns to leadership.
  • Undertake additional duties as assigned.

QUALIFICATIONS:

  • Must possess a current Licensed Practical Nurse license in Michigan.
  • Experience in a nursing home setting is required.
  • MDS experience is preferred.
  • Strong relationship-building and customer service skills are essential.
  • Familiarity with Utilization Review and Discharge Planning processes is necessary.
  • Knowledge of Resource Utilization Groups (RUGS) is beneficial.
  • Willingness to travel between contracted facilities with reliable transportation is required.
  • Flexibility to work varying hours as needed.
  • Excellent organizational and prioritization skills are a must.
  • Ability to perform related duties as required or requested.
  • Creative, detail-oriented, and organized individuals are encouraged to apply.
  • Strong written and verbal communication skills, with a focus on maintaining accurate records.
  • Analytical skills and attention to detail are crucial. Proficiency in computer software for word processing, format development, and spreadsheet management is expected.
  • Ability to work effectively with diverse populations and teams.
  • Understanding of the unique healthcare and social needs of elderly individuals, including common medical conditions and age-related challenges.
