Position Overview:

The Clinical Care Coordinator - Registered Nurse is responsible for overseeing the comprehensive care plan for patients with complex health needs. This role involves acting as a key point of contact between patients, families, and the multidisciplinary healthcare team, ensuring seamless coordination of care across various specialties.

Key Responsibilities:

  • Manage patient records and assist in the preparation of medical histories.
  • Conduct medication reconciliation to ensure accuracy in patient treatment plans.
  • Confirm patient appointments and develop a weekly interdisciplinary schedule.
  • Facilitate collaboration among healthcare professionals to enhance patient care.
  • Build rapport with patients and their families to foster trust and communication.
  • Assist with referrals and coordinate follow-up care with social services as needed.
  • Oversee daily patient management and ensure all assessments are completed.
  • Develop standardized templates for electronic medical records across multiple systems.
  • Connect patients with additional resources, including social work and nutrition services.
  • Review specialist notes to streamline appointment scheduling and avoid redundant services.

Qualifications:

Applicants must possess an Associate's degree in Nursing along with a minimum of five years of relevant clinical experience. A Bachelor's degree in Nursing is preferred. Candidates must hold a current nursing license or be eligible for licensure as a Registered Nurse. Basic Life Support (BLS) certification is required, with the expectation to obtain it within the first month of employment. Depending on departmental needs, Advanced Cardiac Life Support (ACLS) and/or Pediatric Advanced Life Support (PALS) certifications may also be necessary.

Skills and Competencies:

Successful candidates will demonstrate strong written and verbal communication abilities, as well as effective interpersonal skills. The ability to work independently and manage time efficiently is essential.
