Job Description

Job Summary:

Christ Health Center Inc is seeking a highly skilled Mental Health Services Coordinator to join our team at Live Oaks, our Behavioral Health Clinic. As a key member of our team, you will be responsible for coordinating mental health services for patients with behavioral health disorders under the direction of our staff psychiatrist.

Key Responsibilities:

  • Mental Health Service Coordination: Maintain ongoing contact between patients and providers to facilitate treatment and care for patients needing behavioral health services.
  • Client Intake and Assessment: Gather mental health history, medication history, and referrals to facilitate scheduling and patient care.
  • Administrative Tasks: Assist with completing disability forms, ordering laboratory supplies, and maintaining accurate records.
  • Communication and Collaboration: Develop and maintain constructive relationships with patients, families, and healthcare providers to ensure seamless care coordination.
  • Professional Development: Participate in in-service education, quality assurance programs, and risk-management initiatives to enhance clinical skills and knowledge.

Requirements:

  • Education and Experience: Bachelor's or Master's degree in Social Work or Nursing, with 1 year of experience in psychiatric or behavioral health care.
  • Skills and Abilities: Excellent communication and interpersonal skills, analytical skills, attention to detail, and ability to work in a fast-paced environment.
  • Certifications and Licenses: Current CPR certification and ability to obtain necessary certifications and licenses.

Working Conditions:

  • Physical Demands: Ability to lift objects weighing up to 20 pounds, sit for extended periods, and perform manual tasks.
  • Environmental Factors: Work in a multi-cultural setting, with potential exposure to airborne pathogens.

Language Skills:

Ability to read, write, and communicate effectively in English, with proficiency in Microsoft Office and Electronic Health Records (EHR) systems.
