Care Coordinator for Pediatric Nutrition Services

Position Overview

The Registered Dietitian Care Coordinator plays a crucial role in assessing, planning, facilitating, and advocating for optimal clinical and financial outcomes for pediatric patients.

Working closely with the attending physician, family members, and the broader healthcare team, this role is responsible for coordinating individualized care plans and effectively communicating these plans to all relevant parties.

The Care Coordinator also aids in the judicious use of resources, both within the healthcare facility and in external environments.

Collaboration may involve coordinating admissions with referring healthcare providers, conducting utilization reviews, planning for discharges, arranging transportation, and overseeing comprehensive care coordination.

The aim is to ensure that high-quality care is delivered in a manner that is both efficient and cost-effective.


Qualifications

Registered Nurse:

A Bachelor of Science in Nursing (BSN) is preferred.

Consideration Given To:

An Associate Degree in Nursing (ADN) accompanied by a minimum of three years of recent nursing experience, with a strong preference for at least one year in pediatric nursing. Relevant nursing experience may encompass inpatient care, case management, utilization review, discharge planning, and community health nursing. Experience in utilization review and/or discharge planning is advantageous.



Registered Dietitian:

A Master's degree in food and nutrition from an accredited program is preferred, ideally with at least 12 months of clinical nutrition experience, particularly in pediatrics, and certification in a specialized area.

Alternatively, a Bachelor's degree in nutrition from an accredited program is required, along with two years of clinical nutrition experience, with pediatric experience being preferred. Completion of a pediatric nutrition traineeship is beneficial but not mandatory.
