YOUR ROLE
As a Patient Care Coordinator at Evergreen Nephrology, you will oversee a designated group of patients and, in collaboration with our nephrology partners and interdisciplinary care team, assist patients in achieving their kidney care and overall health objectives.
Your responsibilities will include:
- Building trusting, supportive relationships with patients and their families.
- Engaging with a panel of adult patients in a clinical environment alongside interdisciplinary professionals, including nephrologists, nurse care managers, and social workers.
- Monitoring inpatient and skilled nursing facility admissions, coordinating care, and ensuring timely follow-ups.
- Collaborating closely with nurse care managers on outpatient care coordination, including home health services and referrals.
- Taking accountability for the health of a patient population and ensuring they are up to date on preventive measures.
- Communicating effectively with patients and escalating concerns to the care team as needed.
- Connecting patients with community resources and coordinating care across the kidney care continuum.
- Participating in the development of pragmatic care plans and capturing relevant health information.
