We are seeking an experienced Hospice Triage LPN or RN to join our team on weekends, providing vital support to patients, families, and clinical staff. As a key member of our team, you will be responsible for managing patient inquiries, coordinating referrals, and ensuring compliance with Medicare Conditions of Participation.

This full-time role requires a strong organizational skillset, excellent communication skills, and the ability to multitask in a fast-paced environment. If you have 2+ years of experience in an office setting, preferably in home health, and possess a passion for hospice care, we encourage you to apply.
