Position Overview:
The Patient Care Coordinator plays a crucial role in providing administrative support to clients of the Bridgewell Clinic.

Work Schedule:
Monday - Friday, with flexible hours between 8:00 AM - 4:00 PM or 9:00 AM - 5:00 PM (40 hours per week).

Key Responsibilities:
1. Welcome and assist visitors while directing phone inquiries to the appropriate staff.
2. Manage scheduling for intake and follow-up appointments in the Electronic Health Record (EHR) system for all prescribers.
3. Facilitate new intakes in the EHR for salaried clinicians as needed.
4. Support the Intake/Registration Coordinator by ensuring patient insurance cards are scanned into the EHR.
5. Collect and update essential patient information, including contact details and insurance coverage, during scheduled appointments.
6. Ensure the clinic operates smoothly by sorting incoming mail, scanning client documents into the EHR, and processing medication refills daily.

7. Review lab results daily, prioritizing them for scanning by prescribers.
8. Maintain updated releases of information and handle requests for information as necessary.
9. Keep reception, conference rooms, kitchen, and waiting areas organized and stocked with supplies.
10. Collect co-pays as applicable and accurately record them in the EHR.
11. Prepare documentation for the monthly Utilization Review process.
12. Monitor and renew PT-1s (Prescriptions for Transportation) as needed.
13. Provide coverage at other Bridgewell Outpatient Behavioral Health Clinics when required.
14. Process inactive or terminated clinic cases and maintain a record of client deaths annually.

Additional Duties:
Perform other responsibilities as assigned by the supervisor.

Qualifications:
1. High School diploma is required; experience with medical records and office operations is preferred.
2. Ability to work collaboratively and communicate effectively within a multidisciplinary team.
3. Bilingual candidates are preferred.

Required Skills:
1. Excellent written and verbal communication skills.
2. Strong problem-solving abilities and organizational skills.
3. Proficiency in Microsoft Word and Excel is essential.
4. Attention to detail and ability to manage multiple tasks effectively.
5. Exceptional customer service skills, with a patient and sensitive approach in all interactions.

Physical Requirements:
The physical demands of this position require the ability to perform essential functions, with reasonable accommodations available for individuals with disabilities. Frequent stair climbing and the ability to lift up to 20 pounds are necessary. A normal range of eyesight and hearing, with or without corrective measures, is also required.
