Job Summary

We are seeking a highly skilled and motivated Care Coordinator to join our team at Maple Ridge Manor. As a key member of our Resident Care Services department, you will play a vital role in ensuring the highest level of care and service for our residents.

Key Responsibilities
  • Coordinate resident care related to medications, working closely with all departments, medical professionals, families, and administrative staff to provide continuity and adherence to scope of practice and licensure.
  • Supervise the medication room, pass techniques, documentation, and provision of care related to medication, ensuring compliance with state regulations.
  • Provide mentorship to new Medication Aides and lead by example in maintaining a safe and efficient medication administration process.
  • Coordinate physician and other medical appointments, ensuring effective communication and follow-up with the Health and Wellness Director.
  • Monitor and audit daily documentation, including medication administration records, communication logs, and chore completion, to ensure accuracy and compliance.
  • Lead the team in maintaining accurate and up-to-date resident charts, including medication lists, physician orders, and incident reports.
  • Collaborate with the Health and Wellness Director to ensure seamless communication and coordination of resident care.
  • Participate in on-call rotations, providing support and coverage as needed, and responding to emergency situations in a timely and professional manner.
  • Conduct regular medication cart audits, ensuring compliance with regulations and reporting any discrepancies to the Health and Wellness Director.
  • Monitor stock supply levels and report any discrepancies to the Health and Wellness Director or Administrator.
  • Act as a point of contact for care staff, Medication Aides, families, and residents, addressing concerns and reporting to the Health and Wellness Director or Administrator as needed.
  • Provide exceptional customer service, responding to inquiries and concerns in a timely and professional manner.
Requirements
  • High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred.
  • Minimum 1-2 years of experience in a healthcare setting, preferably in a medication aide or supervisory role.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse groups.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Strong organizational and time management skills, with the ability to prioritize tasks and meet deadlines.
  • Knowledge of state regulations and guidelines related to medication administration and resident care.
