Position Summary:
The Patient Care Coordinator plays a vital role in providing comprehensive administrative support to clients of the Bridgewell Clinic.

Work Schedule:
Standard hours are Monday through Friday, with options for 8:00 AM - 4:00 PM or 9:00 AM - 5:00 PM.

Key Responsibilities:
- Welcome and assist visitors, directing phone inquiries to the appropriate staff.
- Manage scheduling for intake and follow-up appointments within the Electronic Health Record (EHR) system for all prescribers.
- Facilitate new intakes in the EHR for salaried clinicians as necessary.
- Collaborate with the Intake/Registration Coordinator to ensure new referrals are processed efficiently, including scanning insurance documentation into the EHR.
- Collect and update essential client information during appointments, ensuring accuracy in the EHR.
- Support clinic operations by sorting incoming mail, scanning client documents, and processing medication refills daily.
- Ensure lab results are reviewed and prioritized for scanning by prescribers.
- Maintain updated records, including releases of information and patient contact details, while handling requests for information as required.
- Organize reception, conference, kitchen, and waiting areas, and manage office supply inventory.
- Process co-pays and record transactions in the EHR.
- Prepare documentation for monthly Utilization Review processes.
- Monitor and renew transportation prescriptions as needed.
- Provide coverage for other Outpatient Behavioral Health Clinics when necessary.
- Manage inactive or terminated clinic cases and maintain records of client deaths.

Qualifications:
- High School diploma is required; experience in medical records and office operations is preferred.
- Strong communication skills and the ability to work collaboratively within a multidisciplinary team are essential.
- Bilingual candidates are preferred.

Skills and Knowledge:
- Excellent written and verbal communication abilities.
- Strong problem-solving and organizational skills.
- Proficiency in Microsoft Word and Excel is required.
- Attention to detail and the ability to multitask effectively.
- Exceptional customer service skills, with a focus on patience and sensitivity in all interactions.

Physical Requirements:
The physical demands of this role require the ability to climb stairs and lift/move up to 20 pounds. Employees must have a normal range of vision and hearing, with or without corrective devices.
