Position Title: HIV Counselor and Retention Specialist

Department/Division: Medicine/ HIV Medicine

Reports To: Supervisor of Case Management/ Manager of Client Services

Key Responsibilities:

As an HIV Counselor, you will provide essential counseling and testing services both onsite and within the community. Your role will involve engaging new patients and those who have previously disengaged from care, ensuring they receive the necessary support and resources.

Core Duties Include:

  • Conducting HIV counseling and testing at designated community events, adhering to established protocols.
  • Participating in outreach initiatives to promote awareness and access to services.
  • Delivering pre- and post-test counseling for clients, whether scheduled or walk-ins, following established procedures.
  • Identifying signs of HIV sero-conversion and collaborating with medical providers to ensure best practices are upheld.
  • Acting as a liaison with community organizations regarding counseling and testing services.
  • Maintaining accurate records and educational materials for clients undergoing testing.
  • Facilitating the completion of HIV reporting and Partner Notification forms, ensuring compliance with state regulations.

Patient Linkage and Care Coordination:

Link patients to HIV care within 30 days of confirmatory testing, coordinating with both community and internal care teams to facilitate access. Collaborate with multidisciplinary teams to ensure timely linkage to care for newly diagnosed patients.

Data Management:

Complete necessary eligibility forms and document all patient interactions in the electronic health record. Maintain accurate records to support grant requirements and quality assurance activities.

Qualifications:

A Bachelor’s degree in social work, psychology, or sociology is required. Prior experience in a healthcare environment is preferred.

Skills:

Strong communication skills, both written and verbal, are essential. Proficiency in medical terminology and the ability to understand complex medical information is beneficial. Fluency in English is required; knowledge of Spanish is an advantage.

Physical Demands:

This position may require occasional lifting and moving of light objects. The work environment is generally moderate in noise level and may involve exposure to infectious diseases.

Salary Range: $60,000 - $96,492.00
