Position Overview:
The Case Manager for the Eating Disorders Program plays a crucial role in bridging the gap between patients and their families, ensuring a smooth transition of services and the provision of timely and effective treatment interventions.

Key Responsibilities:
As a vital member of the Northeast Comprehensive Care Center for Eating Disorders (NECCCED), the Case Manager is responsible for:

  • Conducting thorough assessments and screenings to identify challenges faced by clients, including those related to illness adjustment, employment, and housing.
  • Gathering pertinent information regarding medical, psychological, and social factors that impact the client's situation, while evaluating these elements in relation to the client's capabilities.
  • Providing crisis intervention and support, prioritizing the safety of both the client and themselves, while adhering to legal responsibilities concerning domestic violence and abuse.
  • Facilitating access to essential resources to achieve patient outcomes through effective case management.
  • Analyzing patient care needs and employing a systematic approach to care delivery.
  • Performing telephone triage, coordinating care, and maintaining accurate documentation.
  • Collaborating with healthcare professionals to develop educational materials for patients and their families.
  • Offering individualized counseling to clients and their support systems, empowering them to utilize their strengths and community resources for improved functioning and independence.
  • Addressing the emotional and psychological challenges associated with life-threatening illnesses, while considering diversity in interventions.
  • Providing education to patients and families, taking into account cultural, religious, and motivational factors.
  • Coordinating multidisciplinary care by working closely with various departments within the organization.
  • Facilitating case conferences as needed to ensure comprehensive service delivery.
  • Screening clients for eligibility in entitlement programs and referring them to appropriate services.
  • Maintaining accurate records of client and community resource information.

Qualifications:
To excel in this role, candidates should possess:

  • A Bachelor's degree in Psychology, Sociology, or Social Work; a Master's degree is preferred.
  • 1 to 3 years of documented experience in providing case management services to medically compromised populations.
  • Familiarity with community resources and the ability to work collaboratively within an interdisciplinary medical team.

Salary Range:
$65,000 - $107,343.00
