About the Role

Catholic Charities of the Diocese of Albany is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in ensuring the well-being of our clients by assessing and developing care plans to address gaps in care and social determinants of health.

Key Responsibilities:

  • Assess and develop care plans to assist clients in addressing gaps in care and social determinants of health
  • Coordinate the care of individuals with two or more chronic illnesses
  • Refer and link clients to needed services and resources
  • Work with high-risk populations, including those with mental health diagnoses, homelessness, HIV/AIDS, Sickle Cell Anemia, and/or Substance Use Disorder
  • Ability to work in a fast-paced environment
  • Complete proper documentation

Requirements:

  • Bachelor's Degree (preferred but not required) in Health, Education, or Human Services
  • 1-3 years of case management experience
  • Experience working with diverse populations
