Compensation: Salary commensurate with experience, ranging from $65,057 to $107,343.00
Position Overview:
The Clinical Care Coordinator is responsible for leading the interdisciplinary team in the planning, coordination, implementation, and evaluation of patient care across various service lines. This role emphasizes proactive collaboration with Quality Improvement Teams, adherence to patient care standards, and effective utilization management to optimize clinical and financial outcomes.
Key Responsibilities:
1. Collaborate with the admission physician and interdisciplinary teams to ensure seamless care coordination throughout the patient’s journey in the hospital.
2. Monitor clinical processes diligently to guarantee timely interventions that lead to optimal patient outcomes while adhering to financial constraints.
3. Work closely with primary nursing staff to assess discharge planning needs, coordinate resources, and evaluate the effectiveness of discharge strategies.
4. Initiate discharge planning at the time of admission to enhance continuity of care.
5. Engage with the healthcare team to manage care effectively across the continuum, including pre-admission, discharge, and post-discharge phases.
6. Utilize specialized knowledge and evidence-based guidelines to guide the healthcare team in developing individualized multidisciplinary care plans.
7. Facilitate care conferences for patients with complex health issues, ensuring comprehensive care management.
8. Provide education to patients and families to support the discharge process and promote continuity of care.
9. Review admission data to confirm diagnoses, establish appropriate lengths of stay, and identify potential outliers.
10. Communicate with payers to confirm benefits and conduct concurrent reviews.
11. Identify high-risk patients for collaboration with financial services to address resource availability.
12. Ensure compliance with medical/legal documentation standards in patient records.
13. Collaborate with the healthcare team to implement strategies aimed at reducing length of stay and resource consumption.
14. Assess educational needs and provide training opportunities for healthcare professionals relevant to specific patient care scenarios.
Qualifications:
- Bachelor's Degree required; Master's Degree preferred.
- 1-3 years of clinical experience in a relevant service area required.
- Recent experience in case management, utilization management, or discharge planning in a high-volume acute care setting preferred.
- Strong communication, facilitation, and organizational skills.
- Ability to think critically and solve problems creatively.
- RN licensure required; Certified Case Manager and PRI preferred upon hire.
Physical Requirements:
- Frequent standing, walking, lifting, and carrying.
- Occasional pushing, pulling, climbing, and balancing.
- Constant communication and coordination activities.
