POSITION SUMMARY

This role is part of a supportive housing initiative aimed at assisting individuals residing in permanent scattered-site accommodations. Under the guidance of the Program Director, the Peer Specialist acts as a mentor, providing education on self-help strategies, effective coping mechanisms derived from personal experiences, and skills for managing symptoms. The Peer Specialist plays a crucial role in helping individuals define their rehabilitation and recovery objectives while fostering the development of community support systems and networks.

KEY RESPONSIBILITIES:

1. Provide counseling to individuals regarding their health and mental health needs, empowering them to utilize their inner strengths and available resources to maintain independence within the community.

2. Educate clients on self-help techniques to promote social and community integration.

3. Share effective coping strategies based on personal lived experiences.

4. Instruct on symptom and medication management.

5. Assist in defining rehabilitation and recovery goals.

6. Maintain accurate and up-to-date case records for each individual.

7. Gather and develop resource information for clients, ensuring clarity and comprehensibility in communication.

  1. Support individuals in achieving personalized service plan goals with a Whole Health approach.
  2. Facilitate tailored services that empower individuals to thrive within the community.

10. Refer individuals to internal services, community resources, and external organizations, accompanying them to medical appointments and treatment facilities as needed.

11. Advocate for individuals with external service providers and within the organization.

12. Assist individuals in accessing vocational training and other educational opportunities, collaborating with team members to address individual needs.

13. Provide health and mental health support while promoting social and community integration.

14. Adhere to attendance and timekeeping policies, consistently reporting to work.

15. Participate in scheduled clinical meetings, staff meetings, and training activities.

16. Follow agency infection control policies.

17. Perform other job-related duties as assigned.

REQUIRED SKILLS AND QUALIFICATIONS:

- Ability to engage with residents, families, and staff in a compassionate and respectful manner, recognizing cultural differences.

- Serve as a positive role model for individuals.

- Proficient in completing written documentation accurately and promptly.

- Effective communication skills with staff, residents, families, and the public.

- Ability to prepare timely reports and other written materials as required.

- Capability to collaborate effectively with others.

- Independent worker who adheres to safety and accountability standards.

- Understanding of how disabilities impact functioning and strategies to cope with these challenges.

- Literacy at a 12th-grade level, with the ability to follow written and verbal instructions.

- Willingness to accompany or transport residents as necessary.

- Empathetic and supportive approach to instructing and encouraging appropriate behaviors and skills.

EDUCATION AND EXPERIENCE:

Preferred completion of an internship or similar training in peer advocacy; experience in providing advocacy services to individuals with mental health challenges or those experiencing homelessness.

Preferred past or current recipient of mental health, substance use, or homeless services.
