Job Summary

CharterCARE Health Partners is seeking a highly skilled Referral Coordinator to join our team. As a Referral Coordinator, you will play a critical role in ensuring the quality of the referral process is upheld to departmental and system standards.

Key Responsibilities
  • Develop and maintain collaborative relationships with providers and outpatient practices to ensure accurate and timely referrals.
  • Review incoming referrals for accuracy and completeness, and obtain necessary authorizations and information from patients.
  • Monitor and track referrals to ensure quality control and meet targets.
  • Provide exceptional customer service to patients and their families, and develop strong relationships with healthcare providers.
Requirements
  • High School Diploma or equivalent required; advanced medical degree/certification preferred.
  • Bilingual Portuguese/Spanish required/strongly preferred.
  • Four plus years of relevant work experience in a medical/physicians office required.
  • Previous knowledge and experience processing online/phone authorizations required.
  • Previous coding/billing experience preferred.
Preferred Qualifications
  • Knowledge of basic software applications, including Electronic Health Record management.
  • Experience working with Athena and knowledge of Meditech and Infina.
  • Strong interpersonal and customer relations skills.
  • Ability to lead multiple staff/disciplines and handle multiple priorities with minimal supervision.
  • Ability to comprehend and follow written and verbal instructions, and maintain confidentiality of patient and employee information.
