Job Summary

The Columbus Organization is seeking an experienced Support Coordinator to join our team in Newark, DE. As a Support Coordinator, you will play a vital role in assisting individuals with intellectual challenges and developmental disabilities.

Key Responsibilities

  • Assess and evaluate the needs of individuals with intellectual challenges and developmental disabilities
  • Develop and implement personalized service plans to meet the unique needs of each individual
  • Collaborate with healthcare professionals, family members, and other stakeholders to ensure comprehensive support
  • Provide advocacy and support to individuals in accessing community resources and services
  • Maintain accurate and up-to-date records of individual progress and service plans

Requirements

  • Minimum of one year experience in health or human services support
  • Six months experience in case management and service planning
  • Six months experience in making recommendations for client service plans
  • Six months experience in interpreting laws, rules, and regulations
  • Six months experience in using automated information systems
  • Six months experience in narrative report writing
  • Valid state driver's license and access to a vehicle
  • Associate's degree preferred, but not required
  • Bilingual (Spanish) preferred, but not required
