About the Role:We are seeking a highly skilled Registered Nurse to serve as a Care Coordinator in our Enola, Pennsylvania location.
Job Responsibilities:- Navigate Patient Journey: Facilitate patient care throughout the healthcare system upon discharge disposition.
- Support Staff and Coordinate Care: Collaborate with staff to ensure seamless transitions of care across the outpatient healthcare setting.
- Recommend Alternative Locations of Care: Identify appropriate services or providers for referrals and recommend alternative locations of care when necessary.
- Licensure: Current licensure to practice as a Registered Nurse by state of employment or multistate RN license through the interstate Nurse Licensure Compact.
- Certification: AHA BLS certification prior to end of orientation period.
