About the Role

This role aims to facilitate the patient journey through proper navigation of the healthcare system upon discharge disposition. The successful candidate will support staff in coordinating care while patients transition through the healthcare continuum.

The Care Coordinator will serve as a clinical resource to scheduling coordinators and assist in identifying appropriate services or providers for referrals. Additionally, they will recommend alternative locations of care when necessary and identify discharge needs and services.

Requirements
  • Curently licensed to practice as a Registered Nurse by the state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.
  • AHA BLS certification required before the end of the orientation period.
