Job Summary

The Registered Nurse Navigator will serve as a clinical resource to scheduling coordinators, assist in identifying appropriate services or providers for referrals, and recommend alternative locations of care when necessary. This role requires strong communication skills, attention to detail, and the ability to work collaboratively with various teams.

Key Responsibilities
  • Facilitate patient journeys to ensure proper navigation throughout the healthcare system upon discharge disposition
  • Support staff in coordinating care while patients transition through the healthcare continuum
  • Serve as a clinical resource to scheduling coordinators
  • Assist in identifying appropriate services or providers for referrals
  • Recommend alternative locations of care when necessary
Requirements

To be successful in this role, you must have a current license to practice as a Registered Nurse in the state of employment or hold a multistate RN license through the interstate Nurse Licensure Compact. Additionally, you must obtain AHA BLS certification prior to the end of the orientation period.
