Position Overview:

The Sterling Area Health Center is seeking a dedicated RN Care Manager / Team Lead to oversee a diverse caseload of patients with complex health needs. This role is pivotal in ensuring that patients receive comprehensive care and support throughout their healthcare journey.

Qualifications:
  • Must possess a valid Michigan Registered Nurse license.
  • A minimum of three years of experience in adult and pediatric care settings, including primary care, home health, skilled nursing, or hospital environments.
  • Strong understanding of chronic health conditions, preventive care, and patient education.

Preferred Qualifications:
  • Bachelor's degree or higher in a clinical discipline.
  • Experience in care management and relevant training.

Supervision:
Reports to the Lead RN Care Manager.

Key Responsibilities:
  • Manage a diverse caseload of patients, implementing targeted interventions to reduce hospitalizations and emergency visits.
  • Coordinate care across various healthcare settings, assisting patients and families in navigating their healthcare options.
  • Conduct assessments and develop individualized care plans that prioritize patient needs and promote optimal health outcomes.
  • Collaborate with healthcare teams, including specialists and community resources, to ensure seamless care transitions.
  • Utilize evidence-based guidelines to enhance care delivery and patient engagement.
