Position Overview:

The Sterling Area Health Center is seeking a dedicated RN Care Manager / Team Lead to oversee a diverse caseload of patients with complex health needs. This role is pivotal in ensuring that patients receive coordinated and comprehensive care tailored to their individual circumstances.

Qualifications:
  • Must possess a valid Michigan Registered Nurse license.
  • A minimum of three years of experience in adult and pediatric care settings, including primary care, home health, skilled nursing, or hospital environments.
  • Strong understanding of chronic illnesses, evidence-based practices, and patient education methodologies.

Preferred Qualifications:
  • Bachelor's degree or higher in a relevant clinical field.
  • Experience in care management and relevant training.

Supervision:
Reports to the Lead RN Care Manager.

Key Responsibilities:
  • Manage a caseload of patients with complex health issues, providing targeted interventions to prevent hospitalizations and emergency visits.
  • Coordinate care across various settings, assisting patients and families in navigating healthcare options.
  • Conduct assessments and develop individualized care plans that align with evidence-based guidelines.
  • Collaborate with primary care teams and specialists to ensure quality and efficient healthcare delivery.
  • Empower patients and families through self-management support, enhancing their capacity for self-care.
  • Implement monitoring systems for high-risk patients to facilitate early intervention during health crises.
  • Maintain comprehensive documentation of care management activities and continuously evaluate processes for improvement.

  • Exceptional interpersonal skills to effectively engage with patients, families, and healthcare professionals.
  • Strong leadership capabilities, including time management and problem-solving skills.
  • Ability to analyze complex data and manage clinical issues independently.
  • Proficient in utilizing technology for documentation and communication.
