Qualifications:
- Current Michigan Registered Nurse license required.
- Minimum of three years of experience in adult and pediatric care within primary care, home health, skilled nursing, or hospital settings over the past five years.
- Strong understanding of chronic conditions, evidence-based practices, wellness, health risk assessments, and patient education.
Preferred Qualifications:
- Bachelor's degree or higher in a clinical discipline.
- Experience and training in care management.
Supervision:
- Direct supervision of the RN Care Manager.
Key Responsibilities:
- Manage a diverse caseload of patients with complex health needs.
- Implement targeted interventions to minimize hospitalizations and emergency visits.
- Facilitate care coordination across various healthcare settings, assisting patients and families in navigating healthcare options.
- Conduct infrequent home visits as necessary.
- Provide comprehensive care management for both adult and pediatric patients under minimal supervision.
- Collaborate with primary care leadership to enhance care delivery, ensuring quality and cost-effective services.
- Assess, plan, implement, coordinate, monitor, and evaluate patient care options to optimize health outcomes.
- Develop individualized care plans based on evidence-based guidelines and protocols.
Major Duties:
1. Identify high-risk patient populations through referrals and risk assessments.
2. Evaluate the healthcare, educational, and psychosocial needs of patients and families.
3. Collaborate with healthcare teams to create and adjust individualized care plans.
4. Empower patients and families through self-management support.
5. Establish systems for monitoring high-risk patients to prevent complications.
6. Implement clinical interventions aligned with evidence-based guidelines.
7. Ensure follow-up care during transitions between healthcare settings.
Documentation and Evaluation:
- Maintain accurate records of all care management activities.
- Work with leadership to assess processes and propose improvements in care management.
- Stay updated on best practices in engagement, communication, and behavior change strategies.
Skills and Abilities:
- Strong interpersonal skills for effective communication with diverse populations.
- Leadership qualities including time management, problem-solving, and critical thinking.
- Ability to build long-term relationships with patients and families while maintaining professional boundaries.
