POSITION SUMMARY:

Serves as a pivotal care coordinator within the hospice environment. Evaluates and determines patient requirements in alignment with the hospice philosophy of care. Collaborates with a multidisciplinary team to deliver comprehensive patient and family support and referrals. Ensures effective pain management and symptom alleviation. Upholds continuity of care by addressing patient needs efficiently.

EDUCATIONAL BACKGROUND AND EXPERIENCE:

  • Must possess an Associate's Degree or Nursing Diploma.
  • Requires a thorough understanding and acceptance of the hospice care philosophy, along with a solid grasp of contemporary pain and symptom management techniques.
  • Demonstrates strong interpersonal skills to positively represent hospice services to potential program referrals.
  • Exhibits flexibility in modifying policies and procedures to cater to the unique needs of hospice patients and their families.
  • Prior experience in hospice care is advantageous.

CERTIFICATIONS AND LICENSES:

  • Must hold a current Registered Nurse license in the State of Michigan.
  • Hospice certification is a plus.
