Clinical Care Manager RN

The Sterling Area Health Center is seeking a dedicated RN Care Manager / Team Lead to oversee a team of care managers and provide exceptional support to patients with complex health needs.
Qualifications:
- Must possess a valid Michigan Registered Nurse license.
- A minimum of three years of experience in adult and pediatric care within primary care, home health, skilled nursing, or hospital settings is required.
- Knowledge of chronic disease management, preventive care, and patient education is essential.
Preferred Qualifications:
- Bachelor's degree or higher in a relevant clinical field.
- Experience in care management and related training.
Supervision:
Reports to the Lead RN Care Manager.
Key Responsibilities:
- Manage a diverse caseload of patients, implementing targeted interventions to minimize hospitalizations and emergency visits.
- Coordinate care across various healthcare settings, assisting patients and families in navigating their healthcare options.
- Conduct assessments and develop individualized care plans that prioritize patient-centered approaches.
- Work collaboratively with primary care teams and specialists to ensure seamless care delivery.
- Provide education and support to empower patients and families in self-management of their health conditions.
- Monitor high-risk patients closely to prevent complications and facilitate timely interventions.
- Maintain accurate documentation of care management activities and patient interactions.
Skills and Abilities:
- Strong interpersonal skills to effectively engage with patients, families, and healthcare professionals.
- Demonstrated leadership abilities, including effective communication, problem-solving, and decision-making skills.
- Ability to analyze complex data and manage clinical issues with critical thinking.
- Capacity to work independently while being accountable for patient care.
- Proficient in using technology and electronic health records.
