Position Overview:

The Sterling Area Health Center is seeking a dedicated RN Care Manager / Team Lead to oversee a diverse caseload of patients with complex health needs. This role is pivotal in ensuring effective care management and coordination across various healthcare settings.

Qualifications:
  • Must possess a valid Michigan Registered Nurse license.
  • A minimum of three years of experience in adult and pediatric primary care, home health, skilled nursing, or hospital settings within the last five years.
  • Familiarity with chronic conditions, preventive care, and patient education is essential.

Preferred Qualifications:
  • Bachelor's degree or higher in a clinical discipline.
  • Experience in care management and relevant training.

Supervision:
Reports to the Lead RN Care Manager.

Key Responsibilities:
  • Manage a caseload of patients requiring complex care interventions.
  • Implement strategies to reduce hospitalizations and emergency visits.
  • Facilitate communication between patients, families, and healthcare providers to navigate care options.
  • Conduct assessments and develop individualized care plans that prioritize patient needs.
  • Monitor patient progress and adjust care plans as necessary.
  • Provide education and support to empower patients in self-management of their health.
  • Ensure timely follow-up post-hospitalization, including medication reconciliation and care coordination.
  • Maintain comprehensive documentation of care management activities.
  • Collaborate with healthcare teams to identify process improvements in care delivery.

Skills and Abilities:
  • Strong interpersonal skills to effectively engage with patients, families, and healthcare professionals.
  • Demonstrated leadership abilities, including time management and effective communication.
  • Critical thinking skills to analyze complex patient data and make informed decisions.
  • Ability to work independently while being accountable for patient care outcomes.
  • Proficient in using technology and electronic health records.
