Qualifications:
- Must possess a valid Michigan Registered Nurse license.
- A minimum of three years of experience in adult and pediatric primary care, home health, skilled nursing, or hospital settings within the last five years.
- Familiarity with chronic conditions, preventive care, and patient education is essential.
Preferred Qualifications:
- Bachelor's degree or higher in a clinical discipline.
- Experience in care management and relevant training.
Supervision:
Reports to the Lead RN Care Manager.
Key Responsibilities:
- Manage a caseload of patients requiring complex care interventions.
- Implement strategies to reduce hospitalizations and emergency visits.
- Facilitate communication between patients, families, and healthcare providers to navigate care options.
- Conduct assessments and develop individualized care plans that prioritize patient needs.
- Monitor patient progress and adjust care plans as necessary.
- Provide education and support to empower patients in self-management of their health.
- Ensure timely follow-up post-hospitalization, including medication reconciliation and care coordination.
- Maintain comprehensive documentation of care management activities.
- Collaborate with healthcare teams to identify process improvements in care delivery.
Skills and Abilities:
- Strong interpersonal skills to effectively engage with patients, families, and healthcare professionals.
- Demonstrated leadership abilities, including time management and effective communication.
- Critical thinking skills to analyze complex patient data and make informed decisions.
- Ability to work independently while being accountable for patient care outcomes.
- Proficient in using technology and electronic health records.
