Job Summary:

We are seeking a skilled Registered Nurse to join our Hospice team as a Care Coordinator. In this role, you will work closely with patients and their families to provide comprehensive care and support.

Key Responsibilities:

  • Assess patient needs and develop individualized care plans in accordance with the hospice concept of care.
  • Coordinate with the interdisciplinary team to ensure seamless care delivery and referrals.
  • Provide pain management and symptom control to patients and their families.
  • Maintain continuity of care by ensuring patient needs are met and addressed.

Requirements:

  • Associate's Degree or Diploma in Nursing.
  • Understanding and acceptance of the Hospice concept of care.
  • Working knowledge of current pain and symptom management.
  • Interpersonal skills to present a positive image of hospice to patients and families.
  • Ability to adapt policies and procedures to meet the needs of patients and families.
  • Previous hospice experience preferred.

Certifications/Licenses:

  • Current licensure in the State of Michigan as a Registered Nurse.
  • Hospice certification preferred.
