Job Summary

We are seeking a highly organized and detail-oriented Care Coordination Specialist to join our team at MedPOINT Management. As a key member of our Case Management department, you will play a critical role in coordinating care for our members, ensuring seamless transitions and optimal health outcomes.

Key Responsibilities
  • Open and close cases in a timely and accurate manner
  • Complete and close tasks efficiently, ensuring all documentation is up-to-date
  • Upload and document pertinent information and documents to member files
  • Update condition codes on AccessPoint to reflect accurate member information
  • Conduct outbound calls to engage members using various communication methods
  • Coordinate care for members with non-clinical needs, ensuring their needs are met
  • Assist with contacting medical networks for coordination of care purposes
  • Ensure all documentation is entered timely and correctly in member case files
  • Update the case managers' tracker accurately and efficiently
  • Assist with outreach to members for transitions of care
  • Request medical records and other required information from provider offices and hospitals
  • Log into the Case Management Phone queue as needed
  • Assist with the distribution of Right fax documents
  • Retrieve Health Risk Assessments from Health Plans
  • Provide support to new onboarding team members
  • Generate correspondence letters and care plans as required
  • Complete trainings provided by MPM and HP
  • Respond to emails within 24 hours
  • Attend all mandatory team huddles and meetings
  • Inform management of any issues gathered from communication
  • Participate in other assignments as required
Requirements
  • High school graduate or GED equivalent
  • Minimum of two years' experience in a call center environment; health plan or medical group call center experience preferred
  • Computer proficiency on Microsoft Word, Excel, and PowerPoint
