Role Summary: Under the guidance of the Manager of Nurse Case Management Services, the Registered Nurse Care Coordinator will deliver medical care management and comprehensive support services to patients. This role ensures that case management services adhere to industry standards and fulfill all contractual obligations. The Care Coordinator, as a Registered Nurse, will collaborate with team members to execute, oversee, and sustain a program in alignment with DHSP guidelines. Responsibilities include initial nursing assessments, chronic disease management, referral processes, and facilitating patient access to primary healthcare.

Key Responsibilities:

  • Participate as a member of a multidisciplinary care team to deliver a variety of patient care services.
  • Work alongside healthcare providers and case management staff to design and implement patient care plans for chronic disease management (e.g., HIV, Hepatitis C, Diabetes).
  • Supervise and support LVN Nurse Case Managers.
  • Make independent and significant decisions regarding patient care.
  • Conduct outreach to ensure clients are informed about available services.
  • Collaborate with team members to develop and maintain an effective Utilization Management Program.
  • Assist in implementing specialty referral guidelines and determining patient eligibility for referral authorization.
  • Coordinate specialty referrals according to clinic protocols.
  • Refer clients to ancillary team members, such as Housing Case Management Staff and Nutritionists.
  • Consult with staff on service coordination and service plans for clients.
  • Ensure continuity of care and follow-up for clients.
  • Act as a liaison between specialist consultants and Health Services.
  • Develop educational materials for clients, partners, and family members.
  • Maintain clear communication through case presentations, progress notes, and reports.
  • Oversee coordination with external agencies for services like home health and hospice care.
  • Monitor specialist appointments to ensure compliance with funding and reporting requirements.
  • Provide skilled nursing care, including administering medications and treatments.
  • Conduct assessments in accordance with regulatory guidelines.
  • Assist in training and mentoring staff.
  • Triage patients as necessary.
  • Maintain accurate documentation of all care and services provided.
  • Engage professionally with health services staff and patients.
  • Utilize standard office equipment and software effectively.
  • Participate in meetings and comply with regulatory requirements.
  • Perform additional duties as assigned.

Qualifications:

  • Experience working with the LGBTQ community and understanding relevant issues.
  • Passion for the organization's mission to improve the lives of LGBTQ individuals.
  • AIDS Care Registered Nurse (ACRN) Certification preferred.
  • Current California Registered Nurse License required.
  • BSN or MSN preferred.
  • Current AHA CPR Certification.
  • Minimum of three years of direct patient care experience, preferably in an ambulatory care setting.
  • At least two years of experience in Case Management.
  • Familiarity with HIV/AIDS, Hepatitis C, and chronic disease management.
  • Knowledge of quality assurance and regulatory standards.
  • Strong communication and organizational skills.
  • Proficient in Microsoft Office and Electronic Health Records.
  • Ability to work independently and manage multiple tasks.
  • Bilingual (English/Spanish) skills are a plus.
  • Demonstrated ability to work effectively in a diverse environment.
