- Referral Management: Oversee all outgoing and incoming referrals, ensuring accurate updates to medical providers and maintaining comprehensive records.
- Client Engagement: Communicate with families to discuss service requests, next steps, and gather feedback on client outcomes.
- Database Oversight: Manage the Master Client and Service Delivery databases, along with monthly referral activity reports.
- Care Planning: Collaborate in developing and updating client care plans and participate in case conference meetings.
- Support Group Coordination: Organize activities for caregiver and parenting support groups, including supervision of facilitators and outreach efforts.
- Data Collection: Assist in data gathering and evaluation planning to support program effectiveness.
- Additional Support: Provide assistance to leadership in various aspects of the initiative as needed.
