Job Title: Intake Coordinator

St. Croix Hospice is seeking a highly skilled Intake Coordinator to join our team. As an Intake Coordinator, you will play a critical role in ensuring seamless patient transitions from referral to home care.

Key Responsibilities:
  • Verify client funding sources and work with providers and funding sources to obtain authorizations for service.
  • Collect accurate information for billing and ensure compliance with state, federal, and CHAP referral/intake regulatory requirements.
  • Establish and maintain positive working relationships with current and potential referral sources.
  • Coordinate patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
  • Monitor community and customer perceptions of St. Croix Hospice as a high-quality provider of services.
  • Ensure maximum third-party reimbursement through direct oversight of insurance verification and authorization processes.
  • Maintain comprehensive working knowledge of St. Croix Hospice contractual relationships and ensure patients are admitted according to contract provisions.
  • Participate in quality assessment performance improvement teams and activities.
  • Maintain comprehensive working knowledge of community resources and assist referral sources in accessing community resources.
Requirements:
  • High School Diploma or Equivalent; Associate Degree in Business, Office Administration preferred.
  • Two (1) + years of experience in a healthcare setting, billing, and familiar with medical terminology; insurance knowledge and previous admission experience preferred.
  • Strong communication skills, verbal and written.
  • Working knowledge of office equipment, including fax, printer, photocopier, and voice mail.
  • Strong computer skills with knowledge of Microsoft Word and Excel.
  • Ability to deal tactfully with customers and the community.
  • Demonstrates good communications, negotiation, and public relations skills.
  • Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
