Job Summary

We are seeking a highly skilled and detail-oriented Intake Coordinator to join our team at St. Croix Hospice. As a key member of our admissions team, you will be responsible for ensuring a seamless patient intake process, maintaining positive relationships with customers and referral sources, and managing insurance verification and authorization processes.

Key Responsibilities
  • Receive and verify patient referrals, ensuring accurate information for billing purposes.
  • Work with providers and funding sources to obtain authorizations for service, ensuring compliance with state, federal, and CHAP referral/intake regulatory requirements.
  • Establish and maintain positive working relationships with current and potential referral sources, promoting St. Croix Hospice as a high-quality provider of services.
  • Coordinate patient education and preparation for home care, ensuring a smooth transition to our services.
  • Monitor community and customer perceptions of St. Croix Hospice, identifying opportunities for improvement.
  • Ensure maximum third-party reimbursement through direct oversight of insurance verification and authorization processes.
  • Participate in quality assessment and performance improvement teams, contributing to the development of best practices.
Requirements
  • High school diploma or equivalent; associate degree in business or office administration preferred.
  • At least two years of experience in a healthcare setting, with knowledge of medical terminology and insurance processes.
  • Strong communication and interpersonal skills, with the ability to work effectively with customers, providers, and referral sources.
  • Proficiency in Microsoft Office, including Word and Excel.
  • Ability to work autonomously, with a high level of organization and attention to detail.
