Position Overview

About Us: The ALS Association is a leading national nonprofit organization dedicated to combating ALS through various initiatives. Since its inception, the Association has focused on advancing research, providing support to individuals affected by ALS, and fostering partnerships to enhance the quality of life for patients and their families.

Role Summary: The Manager of Care Services plays a crucial role in establishing and nurturing professional and community relationships to ensure comprehensive care and support for individuals living with ALS. This position oversees the care services program, ensuring consistent and high-quality service delivery for patients and their families. The Manager will also provide education, resources, and navigation assistance to those diagnosed with ALS and their caregivers.


Key Responsibilities:

  • Receive and process referrals, gathering essential patient and medical information to coordinate care plans.
  • Conduct specialized consultations and assessments for patients and caregivers in various settings, including virtual and in-person meetings.
  • Manage a portfolio of patients, ensuring their needs are met effectively.
  • Provide information on available services and educational resources for patients and families.
  • Coordinate referrals to ALS Association programs and community agencies.
  • Facilitate support group meetings, engaging participants and organizing relevant speakers.
  • Collaborate with clinic staff and care services team to maintain strong communication and relationships.
  • Evaluate care services programs for effectiveness and desired outcomes in collaboration with leadership.
  • Ensure adherence to standard operating procedures and accurate reporting within the assigned territory.
  • Assist in the development of strategic partnerships and address operational challenges as needed.
  • Travel as required for the role.
  • Perform additional duties as assigned by management.

Qualifications:

  • Bachelor's Degree in social work, health administration, public health, nursing, or a related field is required; a Master's in Social Work (MSW) is highly preferred.
  • Three to five years of relevant professional experience, including direct care and management.
  • Strong management skills, including problem-solving, critical thinking, and organizational planning.
  • Knowledge of ALS and experience with neurodegenerative diseases is advantageous.
  • Excellent communication skills, both verbal and written, with a focus on confidentiality.
  • Proficiency in Microsoft Office Suite; experience with databases is a plus.
  • Ability to work independently and manage multiple projects simultaneously.
  • Strong customer service orientation and ability to travel as required.

Work Environment:

  • Physical demands include standing, walking, sitting, and occasional lifting of up to 40 pounds.
  • Work may involve exposure to various environmental conditions.
