The Licensed Practical Nurse (LPN) plays a crucial role in delivering direct patient care under the guidance of healthcare providers, registered nurses, or clinical coordinators, adhering to the New York State Practice Act. The LPN is responsible for providing essential patient information to the healthcare team, influencing the planning, delivery, and assessment of patient care. With a focus on compassionate care, LPNs respect each patient's individual needs, values, and preferences.

Under the supervision of a registered nurse or licensed independent practitioner, the LPN administers medications and vaccinations, monitors, records, and communicates clinical data related to a patient's health condition. They perform various clinical tasks, including urinary catheterization, tracheal suctioning, and sterile dressing changes, ensuring they possess the necessary training and competency documentation. Additionally, LPNs offer emotional support and educational resources to patients and their families.

Key Responsibilities:

  • Accurately obtain and document patients' vital signs.
  • Record components of patients' medical history.
  • Communicate effectively with patients, visitors, and team members.
  • Assist in in-office procedures under the supervision of licensed healthcare professionals.
  • Conduct diagnostic CLIA waived testing.
  • Perform venipuncture for laboratory blood tests.
  • Manage prescription call-backs within the scope of practice.
  • Prepare patient charts with relevant clinical information.
  • Maintain inventory of medical supplies and ensure examination rooms are well-stocked.
  • Comply with regulatory standards and organizational policies.

Qualifications:

  • High School Diploma or G.E.D. - required.
  • Experience in a patient care environment - preferred.
  • Strong verbal and written communication skills.
  • Ability to present information clearly and respond to inquiries from patients and staff.
  • Licensed Practical Nurse - State Licensure required upon hire.

Physical Requirements:

  • Constant standing and walking.
  • Frequent lifting and carrying.
  • Occasional climbing and balancing.
  • Frequent stooping, kneeling, and crouching.
  • Constant communication and coordination of movements.

Working Environment:

  • Noise exposure - constant.
  • Frequent hazards present.
