Job Overview

The Client Care Manager plays a crucial role in guaranteeing that our clients receive exceptional support. This position involves creating a personalized Care Plan during the initial client assessment and ensuring it is regularly updated based on ongoing client interactions.

Key Responsibilities:
  • Facilitate client intake sessions and formulate initial Care Plans
  • Manage Client and Caregiver data within scheduling systems
  • Engage in staffing choices to enhance caregiver/client compatibility and Care Plan execution
  • Conduct client visits as necessary
  • Participate in annual evaluations of caregiver performance
  • Attend team meetings and training sessions as required
  • Perform additional tasks as assigned
Required Qualifications:
  • Strong communication, organizational, computer, and time management skills are vital
  • Previous experience in care management is essential
  • Ability to thrive in a dynamic environment with minimal oversight, manage stress effectively, and adapt to changing situations
