Job Summary

Mission Hospital is seeking a skilled Case Management Registered Nurse to join our team. As a Case Manager RN, you will play a critical role in ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimum outcomes.

Key Responsibilities
  • Provide program orientation to patients, families, and caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
  • Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
  • Ensure the implementation of the patient's treatment plan, supporting the patient's strengths, abilities, needs, and preferences.
  • Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, or caregiver.
  • Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
Requirements
  • Minimum of 3 years in nursing practice, with clinical knowledge, IT skills, effective interpersonal communication, critical thinking, successful problem-solving, ability to work autonomously, innovative thinking, and resourcefulness in navigating community resources.
  • Preferred experience in nursing practice and care management, with community or ambulatory care management experience, primary care clinical experience, and experience with inpatient discharge planning work.
  • Required education: Bachelor of Science in Nursing or bachelor's degree in a related field. If RN, effective 3/1/18, Bachelor of Science in Nursing OR enroll in a BSN or advanced degree program within 6 months AND obtain a BSN within 4 years of hire date into RN position.
  • Preferred education: Master's in Nursing.
  • Required license: If RN, must have and maintain current licensure as registered nurse with the North Carolina Board of Nursing.
  • Required certification: BLS Healthcare Provider.
  • Preferred certification: Level I: Maintains nationally recognized certification in Case Management, such as ACM, CCM, or ANA certification.
