As a Post-Acute Care Transition Nurse, you will play a vital role in ensuring that our patients receive high-quality, efficient care. You will collaborate with a team that is committed to supporting one another and prioritizing patient care.
Key Responsibilities:
- Establish and maintain relationships with hospitals, skilled nursing facilities, and community referral sources.
- Provide education and promote collaboration among healthcare providers.
- Conduct screenings and assessments to ensure appropriate patient placement.
- Work closely with interdisciplinary teams to facilitate smooth transitions and prevent readmissions.
- Utilize care management activities to enhance patient outcomes and reduce costs.
Qualifications:
- Bachelor of Science in Nursing or related field.
- Current licensure as a registered nurse with the appropriate board.
- Minimum of two years of experience in a healthcare setting.
- Certification in Case Management is preferred.
