As a Post-Acute Care Transition Nurse, you will play a vital role in ensuring that our patients receive high-quality, efficient care. You will collaborate with a team that is committed to supporting one another and prioritizing patient care.

Key Responsibilities:

  • Establish and maintain relationships with hospitals, skilled nursing facilities, and community referral sources.
  • Provide education and promote collaboration among healthcare providers.
  • Conduct screenings and assessments to ensure appropriate patient placement.
  • Work closely with interdisciplinary teams to facilitate smooth transitions and prevent readmissions.
  • Utilize care management activities to enhance patient outcomes and reduce costs.

Qualifications:

  • Bachelor of Science in Nursing or related field.
  • Current licensure as a registered nurse with the appropriate board.
  • Minimum of two years of experience in a healthcare setting.
  • Certification in Case Management is preferred.
