CarePartners is committed to delivering high-quality patient-centered care. We are seeking a skilled Case Manager RN Inpatient Rehab to join our team.

Key Responsibilities
  • Collaborate with the care management department, service line, and interdisciplinary care team to ensure quality-driven, best-practice care across the continuum of hospitalization.
  • Maximize positive financial outcomes for designated patient populations by intervening at key points in the disease process with post-discharge follow-up phone calls and crucial communication with care providers to promote health compliance and prevent readmissions.
  • Provide designated leadership functions for the Care Management Department.
What You Will Do
  • Provide program orientation to patients, families, and caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
  • Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
  • Ensure the implementation of the patient's treatment plan that supports the patient's strengths, abilities, needs, and preferences, facilitating involvement throughout the rehabilitation process.
  • Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, or caregiver.
  • Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
Qualifications
  • Required Education: RN with Bachelor of Science in Nursing or related field.
  • Preferred Education: BSN or Bachelor degree in related field.
  • Required License: Current licensure as Registered Nurse with the North Carolina Board of Nursing or a compact state as defined by North Carolina Board of Nursing. Current BCLS. Maintains nationally recognized certification in Case Management (ACM or CCM, or ANA approved certification).
  • Preferred License: None
  • Required Experience: Minimum of Five years recent clinical nursing experience in relevant practice area.
  • Preferred Experience: Eight years recent clinical nursing experience in relevant practice area. Five years managerial or Case Management experience. Bi-lingual.
