As a Manager of Case Management, you will play a pivotal role in ensuring patient-centered care by overseeing all aspects of case management within your designated area. You will work closely with the Case Management Director to align with departmental goals and enhance patient outcomes.
Key ResponsibilitiesThe Manager of Case Management Services will:
- Act as a clinical resource and role model for the case management team.
- Supervise and guide staff development while managing daily departmental operations.
- Ensure proficiency in the use of clinical guidelines to optimize patient care.
- Collaborate effectively with physicians, nurses, and interdisciplinary teams to ensure timely patient management.
- Monitor the quality of patient interactions and documentation, conducting regular audits.
- Communicate clearly with patients, families, and healthcare professionals.
- Assume departmental responsibilities in the absence of the Director.
- Manage scheduling and staffing needs within the department.
- Oversee educational initiatives and competency assessments for staff.
- Prepare and submit regular reports as required.
- Seek cost-effective solutions while maintaining high standards of patient safety and care quality.
- Adhere to established policies and regulatory standards, advocating for patient rights and needs.
- Education: Associate Degree in Nursing or Nursing Diploma required; Bachelor's Degree preferred.
- Experience: Minimum of 3 years in clinical nursing; 2 years in a supervisory role preferred.
- Credentials: Active Registered Nurse license in the relevant state.
- Preferred Credentials: Certification in Case Management and/or InterQual.
