Job SummaryWe are looking for a highly motivated and experienced Travel Nurse RN to work in a Hospice setting. As a Hospice Palliative Care Specialist, you will provide high-quality patient care and support to patients and their families in a fast-paced and dynamic environment.
Responsibilities- Provide direct patient care and support to patients and their families in a Hospice setting
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Conduct thorough assessments and evaluations of patients' physical, emotional, and spiritual needs
- Develop and implement effective care plans to meet patients' needs and goals
- Provide education and support to patients and their families on disease management, symptom management, and end-of-life care
- Participate in quality improvement initiatives to enhance patient care and outcomes
To be successful in this role, you will need:
- A current RN license in the state of employment
- At least 1 year of experience in Hospice Palliative Care
- Excellent communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize tasks effectively
- Strong critical thinking and problem-solving skills
