Position Summary
The Patient Care Liaison operates under the supervision of a licensed healthcare professional, executing a variety of clinical and administrative responsibilities within a designated department or clinic at Phoenix Children's Hospital.

This role is primarily administrative but necessitates a foundational clinical knowledge to facilitate patient care, address barriers to access, enhance the overall quality of care, and improve the patient experience.

Key Responsibilities
1. Collaborates with the care team to optimize the patient care journey for families, assisting patients in overcoming obstacles to care, enhancing the quality of service, and improving clinic operations.

2. Serves as a direct point of contact for families within the specified patient population, working in partnership with patients, healthcare providers, nursing staff, referral coordinators, and administrative personnel.

3. Assists the clinical team in completing necessary documentation, including but not limited to:
- School letters
- Forms
- FMLA paperwork
- ROI requests
- CRS applications

4. Gathers data and prepares charts for upcoming appointments, facilitating the scheduling of external specialty tests, receipt of test results, and follow-up on durable medical equipment and specialty medication orders.

5. Works with the team to streamline clinic visits by managing prior authorizations and ensuring families are adequately prepared for their appointments.

6. Identifies patients with care gaps and collaborates with families to ensure timely and appropriate follow-up within the clinic, facilitating appointment scheduling, record requests, and referrals to specialists.

7. Collaborates with physicians to develop protocols aimed at streamlining care while enhancing quality and patient satisfaction.

8. Engages in various activities to ensure readily available and accessible data.

9. Participates in educational programs to maintain current skills and competencies.

10. Performs miscellaneous job-related duties as assigned.
