Job Summary

The Care Coordinator is a critical member of our healthcare team, responsible for ensuring seamless coordination of patient care. This role requires strong communication and organizational skills, as well as a passion for delivering exceptional patient experiences.

Key Responsibilities

  • Telephonic Outreach and Engagement

Our Care Coordinator will engage with patients through phone calls to ensure they receive the necessary care and support. This includes scheduling appointments, coordinating follow-up care, and addressing any concerns or questions patients may have.

Provider Communication

The Care Coordinator will work closely with our healthcare providers to ensure they are aware of patients' needs and can provide the necessary care during appointments.

Scheduling and Coordination

This role involves scrubbing schedules to identify patients with gaps in care and scheduling appointments with the appropriate provider, site, and time.

Electronic Health Records

The Care Coordinator will audit and update patient electronic health records to ensure all necessary information is available for healthcare providers.

Consult Notes and Scheduling

This role requires identifying needed consult notes and contacting providers to obtain and close consults, as well as scheduling consults based on established guidelines.

Attendance and Adaptability

The Care Coordinator is expected to maintain regular and predictable attendance and be adaptable to changing priorities and deadlines.

Requirements

  • Education

A high school diploma or equivalent is required for this role.

Experience

Preferred qualifications include a graduate degree in a medical field, such as a medical assistant program, with at least 2 years of related work experience in Family Medicine or Internal Medicine. Additionally, 1 year of customer service experience is preferred.
