Role Overview

The Community Health Worker (CHW) plays a crucial role in bridging the gap between underserved populations and health and social service systems. This position focuses on building trust and providing encouragement to patients as they navigate their healthcare needs. The CHW assists individuals in accessing community resources tailored to their specific circumstances and collaborates with medical professionals to develop outreach initiatives aimed at promoting and enhancing the health of patients and their families. This role reports directly to the Clinical Leader.

Key Responsibilities

  • Act as a liaison among various service providers, facilitating enrollment in services and community resources while delivering culturally sensitive care.
  • Manage administrative tasks, including completing applications for resources and supporting provider visit documentation.
  • Track and follow up on referrals and outcomes related to pharmacy, durable medical equipment, and home care services.
  • Assist in coordinating transportation, advocate for patients, support clinical staff, conduct health screenings, and facilitate care coordination.
  • Maintain an updated inventory of resources available within the service area across multiple states.
  • Identify and respond to situations requiring mandatory reporting in accordance with state regulations.

Minimum Qualifications

  • A minimum of 2 years of relevant education, experience, or certification.
  • Reliable transportation for commuting to patient locations.
  • Ability to provide in-person care, which may involve physical activities such as standing, walking, and lifting.
  • Access to stable internet connectivity with specified minimum speeds.

Preferred Qualifications

  • Certification as a CHW, CHES, or as a nurse aide or licensed medical assistant.
  • Experience in multicultural environments.
  • Familiarity with Managed Care or Medicaid systems.
  • Previous experience working with kidney disease patients.
  • Proficiency with translation services.
  • Basic computer literacy.
