We seek a candidate with robust leadership capabilities to oversee the field operations of our street medicine and care management teams. The ideal candidate for the Community Health Operations Supervisor role will resonate with our mission of providing equitable healthcare to marginalized communities, ensuring that every individual receives the dignified care they deserve, irrespective of their circumstances. Being part of Wellness and Equity Alliance LLC is not merely a job; it is a commitment to serve those in dire need, directly within their communities.
This role will function as an advocate for community health needs, aiding residents in effectively navigating healthcare systems and social services. Additionally, the supervisor will play a vital role in site leadership, managing logistics and evaluating program effectiveness. Responsibilities may include delivering preventive health services such as screenings and utilizing community health assessments to identify opportunities for expanding clinical services.
Team members involved in this operation will contribute to a sustainable street medicine program aimed at assisting the unhoused populations in Riverside. Individuals experiencing homelessness often face significant barriers to accessing care, and our team is dedicated to fostering trust and reducing these obstacles by providing care directly where it is needed.
Key Responsibilities
- Oversee daily operations and logistics of the program, ensuring alignment with leadership objectives.
- Coordinate team efforts and manage day-to-day activities to meet program goals.
- Supervise the Community Health Worker team, facilitating communication between CHWs, healthcare providers, and operational support.
- Conduct outreach initiatives targeting specific health programs and patient populations, engaging local businesses and organizations.
- Utilize motivational interviewing and coaching techniques to empower patients in achieving their health goals.
- Document participant interactions and maintain comprehensive electronic records, ensuring accurate reporting of activities and outcomes.
- Assist participants in accessing necessary health services and community resources.
- Foster collaborative relationships with diverse stakeholders, enhancing service delivery and reducing barriers.
- Provide health education and promotion tailored to a diverse patient demographic.
- Monitor treatment adherence and support members in navigating their care plans.
Qualifications
- Minimum of one of the following certifications: CHW Certificate, Violence Prevention Professional Certificate, or relevant work experience.
- High School diploma or GED required; an Associate's degree in healthcare, social work, or a related field is preferred.
- A minimum of 5 years of relevant professional or lived experience is necessary.
- Ability to work independently and collaboratively in diverse settings, employing a solution-oriented approach.
- Preference for candidates with Community Support Worker (CSW) or Certified Peer Support Worker (CPSW) credentials.
Preferred Skills
- Experience in military service or public health programs.
- Strong interpersonal skills and ability to follow instructions effectively.
- Knowledge of local community resources and public health initiatives.
- Proficiency in Microsoft Office and Google Business Suite applications.
- Bilingual Spanish speakers are preferred, with a focus on cultural competence in LatinX communities.
