Experience: - Three years of clinical nursing experience in an acute care facility
- Knowledge of nursing practices and procedures
- Strong clinical assessment skills
License: - Current RN License issued by the Oklahoma State Board of Nursing, or a current multistate Compact RN License (eNLC)

Certifications:

  • Current Basic Life Support issued by the American Heart Association required upon hire
  • Comprehensively assess patient goals
  • Monitor patient's progress and adapt medical treatment accordingly
  • Coordinate discharge planning with patients, family, and healthcare team
  • Advocate for patients with scarce resources and third-party payers
  • Knowledge of community resources for patient recovery
  • Strong interpersonal skills for gathering necessary information

Job Description:

  • As an integral member of a multidisciplinary care management team, the nursing case manager plays a crucial role in orchestrating the care and services for designated patient populations.
  • Conduct thorough evaluations of patient objectives, encompassing biophysical, psychosocial, and financial aspects.
  • Continuously monitor patient progress to identify changes in medical requirements and modify treatment plans as necessary.
  • Facilitate discharge planning in collaboration with family members, healthcare professionals, and community resources.
  • Serve as an advocate for patients facing resource limitations, engaging closely with third-party payers to address concerns.
  • Employ robust clinical assessment abilities and an understanding of community resources to support patient recovery effectively.
  • Required educational background includes graduation from an accredited nursing program, with a preference for a Bachelor of Science in Nursing.
  • Strong communication skills are essential for conveying complex information to patients and their families.
