Service Coordinator for Children's Long-Term Support

The Children's Long-Term Support Waiver Service Coordinator plays a crucial role in providing essential social work and case management services to children eligible for the CLTS waiver and their families. This position involves the coordination of a wide range of services, ensuring that clients receive the necessary support in a structured and effective manner.
Key Responsibilities:
1. Under the guidance of the CLTS Program Manager, deliver comprehensive social services and case management to a designated caseload.
2. Maintain a thorough understanding of the unique needs, challenges, and aspirations of assigned clients.
3. Familiarize yourself with the policies and procedures related to funding sources pertinent to your caseload.
4. Conduct assessments, develop plans, and monitor progress in accordance with established protocols.
5. Provide a variety of services, including support, resource identification, data collection, and goal monitoring.
6. Create individualized service plans that specify the types and levels of services required, along with expected outcomes.
7. Collaborate effectively with other agencies and their staff to optimize service delivery and outcomes for clients.
8. Ensure active participation and understanding from all stakeholders involved in the service plan.
9. Advocate for children and families to engage in the assessment and planning processes related to their service needs.
10. Attend agency meetings and training sessions as directed by the RC Coordinator.
11. Prepare necessary documentation, reports, and maintain accurate records as per agency guidelines.
12. Uphold ethical standards in all professional activities as a social worker/case manager.
13. Undertake additional responsibilities as assigned by the Director of Community Resource Center or the CLTS Program Manager.
Qualifications:
- A Bachelor's degree in human services or a related field is required.
- Proficient communication skills, both written and verbal, in English.
- Reliable transportation and a valid driver's license with a clean driving record are essential.
- Must maintain personal auto insurance with specified coverage limits.
- Ability to document case notes and maintain records consistently.
- Willingness to accept and provide constructive feedback within a team environment.
- Capability to navigate within various settings frequently throughout the day.
- Openness to learning and applying treatment philosophies.
- Bilingual skills in Spanish are advantageous.
- Experience working with children with disabilities and their families is preferred.
