Job Summary

We are seeking a highly skilled and compassionate CLTS Waiver Service Coordinator/Mentor to join our team at Kenosha Human Development Services. As a key member of our Community Resource Center, you will provide critical social work and case management services to children with disabilities and their families.

Key Responsibilities
  • Provide social services and case management to an assigned caseload, working closely with families and community resources to ensure the best possible outcomes.
  • Maintain a deep understanding of the special needs, problems, and goals of clients assigned, and develop individualized service plans to meet their unique needs.
  • Collaborate with other agencies and personnel to ensure seamless service delivery and coordination.
  • Act as a mentor and trainer for new staff, providing guidance and support to help them develop the skills and knowledge needed to succeed in this role.
  • Prepare and maintain accurate records and reports, and participate in agency meetings and training sessions as required.
  • Conduct all activities in accordance with accepted standards of ethics for social workers and case managers.
Requirements
  • Bachelor's degree in human services or a related field.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Reliable transportation and a valid driver's license with a good driving record.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Experience working with children with disabilities and their families is highly desirable.
