Position Overview:
The Children's Long-Term Support Waiver Service Coordinator plays a vital role in providing comprehensive social work and case management services to children eligible for the CLTS waiver and their families. This position is essential in ensuring that clients receive a well-coordinated array of services tailored to their unique needs.

Key Responsibilities:
1. Under the guidance of the CLTS Program Manager, deliver social services and case management to an assigned group of clients.
2. Maintain a thorough understanding of the specific needs, challenges, and aspirations of the assigned clientele.
3. Familiarize yourself with the policies and procedures related to available funding sources for your caseload.
4. Conduct assessments, develop plans, and monitor progress in accordance with established protocols.
5. Provide a range of services including support, resource identification, data collection, and progress monitoring.
6. Create individualized service plans that detail the types and levels of services required and the expected outcomes.
7. Collaborate effectively with other agencies and their staff to ensure optimal service delivery and client outcomes.
8. Ensure that all stakeholders have a voice in the planning process and understand the service plan.
9. Advocate for the active involvement of children and families in the assessment and planning processes.
10. Participate in agency meetings and training sessions as directed by the RC Coordinator.
11. Prepare necessary documentation, reports, and maintain accurate records as per agency standards.
12. Conduct all activities in alignment with the ethical standards expected of social workers and case managers.
13. Undertake additional responsibilities as assigned by the Director of the Community Resource Center or the CLTS Program Manager.

Qualifications:
1. A Bachelor's degree in human services or a related field is required.
2. Proficient communication skills, both written and verbal, in English.
3. Reliable transportation and a valid driver's license with a clean driving record are essential.
4. Must carry personal auto insurance with minimum coverage of 100,000/300,000/100,000.
5. Ability to maintain detailed records and document case notes on a daily basis.
6. Capacity to give and receive constructive feedback as part of a collaborative team.
7. Willingness to navigate various environments within the workspace and community throughout the day.
8. Openness to learning and applying treatment philosophies.
9. Bilingual proficiency in Spanish is advantageous.
10. Experience working with children with disabilities and their families is a plus.
